National Patient's Medical Home Expedition

Health Innovation Group offers a National Patient's Medical Home (PMH) Expedition program across Canada and internationally. This program supports the practical implementation of PMH using nine 'high-impact' changes to guide organizations, physicians and their teams through this transformation. Using a multipronged, interdisciplinary approach, family physicians, nurse practitioners and interprofessional teams are supported on their journey toward PMH.
