Practice Facilitation Program

Practice Facilitation Program

As Canada’s leading Practice Facilitation Program, the program builds capacity by training practice facilitators in quality improvement (QI) methods and coaching to introduce, test and implement high-impact changes in primary healthcare.
Format: In-person, virtual or hybrid delivery with 28-32 hours of synchronous learning supported by mentors throughout the program.
Program Credits: Accredited by the Canadian Nurses Association for 28 credits.

Patient’s Health/Medical Home Expedition

Health Innovation Group offers Patient’s Health/Medical Home Expedition for Primary Healthcare Teams across Canada and internationally. This group learning program guides organizations, physicians, nurse practitioners, and their interprofessional teams to achieve Patient’s Health/Medical Home by implementing nine high-impact changes.
Format: Online/Virtual/Hybrid
Program Credits: This program is certified through the College of Family Physicians of Canada for up to 53 Mainpro+ credits.

Teams and Teamwork Series

Teamwork is a core value in healthcare. We know that working together, effectively, as a team can immediately and positively impact patient health outcomes, patient and provider experience.

HIG offers an educational series focused on improving interprofessional teamwork for better care delivery.
