Patient’s Health/Medical Home Expedition for Primary Healthcare Teams

Health Innovation Group offers Patient’s Health/Medical Home Expedition for Primary Healthcare Teams across Canada and internationally. This program supports the practical implementation of PHMH using nine ‘high-impact’ changes to guide organizations, physicians and their teams through this transformation. Using a multi-pronged interdisciplinary approach family physicians, nurse practitioners and interprofessional teams are supported on their journey toward PHMH.
Format: Online/Virtual
Program Credits: This program is certified through the College of Family Physicians of Canada for up to 53 Mainpro+ credits.

Practice Facilitation International Program

The Practice Facilitation International Program is offered in partnership with Health Innovation Action and builds organizational capacity to propel improvements in primary healthcare settings. In this highly interactive program, Health Innovation Action trains practice facilitators to use quality improvement (QI) methods with clinic teams. Practice facilitators introduce and support implementation of high impact evidence-based changes.

Clinic teams identify a priority project and employ practical tools to implement changes aimed at improving access, building reliable tracking systems, enhancing patient care and supporting efficient and resilient healthcare teams.

Format: A virtual program, eight x 3.5 hour sessions, for a total of 28 hours of synchronous learning. Action periods follow each session.

Optimizing Your Practice: Enhanced Programs

HIG’s practice optimization programs introduce focused high-impact changes to test and apply within your practices. You will benefit from interactive, facilitated discussions and peer-to-peer sharing. Exceptional international faculty will provide input to build success for your local practice context.

As one of the foundational high-impact changes for advancing practice optimization, person-centred interactions brings teams together to learn about culturally sensitive, person-centred and trauma-informed care. This educational series is offered by Health Innovation Group, in partnership with Health Innovation Action.

HIG Networks

Advance the change process through shared learning, mentorship, and collaboration with peers! The HIG Networks are intended for those who want to connect with and learn from peers on a similar path. These are offered as regular virtual sessions and an online community platform for sharing.
