Overview
Health Innovation Group offers Patient’s Health/Medical Home Expedition for Primary Healthcare Teams across Canada and internationally. This program supports the practical implementation of Patient’s Medical Home (PHMH) using nine ‘high-impact’ changes to guide organizations, physicians, nurse practitioners, and interprofessional teams on their journey toward PHMH.
Format This program can be delivered virtually or in-person with 12 sessions delivered over 12 months. Each session is 4.5 hours.
