THE LEADING PROGRAM IN HEALTH HOME IMPLEMENTATION

Overview

Health Innovation Group offers a group learning program - the Patient’s Health/Medical Home Expedition for Primary Healthcare Teams across Canada and internationally. This program guides organizations, physicians, nurses and interprofessional teams to achieve the Health Home - also referred to as the Patient’s Medical Home - through the implementation of nine high-impact changes.

During the program teams learn to employ practical tools and resources to improve the healthcare provider experience, strengthen team-based behaviours, improve the patient’s experience and care and build a healthy practice.

Format: This program can be delivered virtually, in-person or in a hybrid format over 12 months. Each session of this series is 4.5 hours. Between the sessions are 1-hour PDSA coaching calls to support teams.
