We are excited to be partnering with the eHealth Centre of Excellence to deliver the Practice Facilitation Program. Practice facilitation is an approach to supporting improvement in primary care practices that focuses on building organizational capacity for continuous improvement.
Overview
This program builds capacity by training practice facilitators in quality improvement (QI) methods and coaching to introduce, test and implement high-impact changes in primary healthcare, through 28-32 hours of synchronous learning supported by mentors throughout the program.
During the program, individuals or teams identify a project and employ practical tools to implement changes to improve access, build reliable tracking systems, enhance patient care, and support efficient and resilient healthcare teams.
