eHealth Centre of Excellence
The eHealth Centre of Excellence works at local, provincial, and national levels to move digital health initiatives forward. Their current suite of digital health tools include, but are not limited to: eConsult and eReferral (through the Ontario eServices Program), virtual care, patient forms, online appointment booking, Evidence2Practice tools and coaching for electronic medical records (EMRs), and automated solutions. They also offer a number of services to Ontario Health Teams, such as integrated care managers, and provide a co-design approach to tool development through their Healthcare Experience and Advancement Lab (HEAL).
Team Primary Care
Canada’s Team Primary Care is an initiative with a vision to develop an integrated health system in which every individual receives equitable, high quality, comprehensive care from a well-trained, well-supported and optimally utilized primary care team.
Correctional Service Canada
Over the past two years, Health Innovation Group has had the privilege of partnering with Correctional Services Canada to enhance integrated primary healthcare delivery, adapting the Patient’s Medical Home model. Through initiatives such as provider learning sessions, capacity-building workshops for quality improvement, new enablers and resources, significant progress has been achieved in advancing evidence-based primary care practices tailored to their specific needs.
New Brunswick Department of Health
The New Brunswick Department of Health is a part of the Government of New Brunswick. It is charged with administration and delivery of public healthcare in New Brunswick.
Health Innovation Action
Health Innovation Action is a dynamic network of transformational leaders and advisors from across Canada. As a not-for-profit organization, HIA fosters partnerships and builds capacity to continuously improve health services.
Women’s College Hospital
With the Women’s College HospitalToronto, HIG provided training to Practice Facilitators to support the uptake of COVID-19 vaccinations in 2021. The project was successfully completed, and led to published results in the Annals of Family Medicine and Practice Facilitation to Support Family Physicians in Encouraging COVID-19 Vaccine Uptake: A Multimethod Process Evaluation.
